Reported for patients 50-75 years of age who had appropriate screening for colorectal cancer. Reported for patients aged 18-75 years with diabetes who had hemoglobin A1c > 9.0% during the measurement period. #001: Diabetes: Hemoglobin A1c (HbA1c) Poor Control (>9%) MDinteractive is a CMS Qualified Registry and is certified by the Office of the National Coordinator for Health Information Technology (ONC) to generate eCQMs used for PCF. The intent of eCQMs is to help measure and track the quality of health care services provided by the clinician or practice. PCF uses one claims-based measure (Advance Care Planning) and three electronic Clinical Quality Measures (eCQMs) that are generated from an EHR.ĮCQMs use data captured from an EHR in structured formats (e.g., data that are entered into specific fields) at the point of patient care. To qualify for a positive performance-based adjustment, practices must meet or exceed average national performance thresholds on a limited set of quality measures that are clinically meaningful for patients. Participating practices must collect and report data for specific quality measures for the entire performance year (January - December). PCF Quality Reporting OverviewĬMS requires annual quality measure tracking and reporting as part of the PCF program. If your electronic health record (EHR) system is unable to aggregate data at the practice site level or if you need to aggregate data across 2 different EHRs, MDinteractive can help by converting your Quality data into the acceptable format. You can find more information about the PCF QRDA III format here. Quality data must be submitted to the Centers for Medicare and Medicaid Services (CMS) in a specified PCF QRDA III format which may be a challenge for some practices. Practices participating in the Primary Care First (PCF) program must meet annual Quality data reporting requirements.
